Physical Therapist (Full-time)

Responsibilities
Conduct assessments and develop individualized treatment plans for patients based on their physical needs and abilities.
Provide in-home physical therapy services to patients in accordance with their treatment plans.
Continuously assess and adjust treatment plans as needed to ensure progress towards patient goals.
Communicate regularly with patients, family members, and other healthcare professionals to coordinate care and ensure effective treatment.
Document all patient interactions, assessments, and treatment plans according to AdvisaCare standards.
Adhere to all state and federal laws and regulations related to physical therapy practice.
Maintain accurate records and reports of patient progress and treatment outcomes.
Requirements
Current licensure as a Physical Therapist in the state of Michigan.
A minimum of one year of PT experience, ideally in a home health care setting, is preferred.
Strong understanding of physical therapy best practices and treatment methodologies.
Excellent communication and interpersonal skills, with the ability to connect with patients and their families.
Ability to work independently and manage a caseload of patients effectively.
Must have a valid driver's license and reliable transportation.
