Job Title: Clinical Lead Nurse
Location: Middlesex Hose, 3rd Floor , 130 College Road, HA1 1BQ
The ideal candidate for this role will be required to mobilize care packages, manage challenging caseloads, and ensure high-quality patient care in line with Care4ocus Healthcare and NMC codes of conduct.
Relationships:
Reports to: Senior Leadership Team, Registered Manager
Supervises: Clinical team, care and support staff
Collaborates with: CCGs, Hospitals, Local Authorities, multidisciplinary teams
Core Duties:
Care Management: Mobilize care packages and manage a challenging caseload. Assess new care packages and collaborate with senior leadership and clinical teams to ensure safety and quality. Rapidly respond to clients’ needs and new care packages. Support end-of-life care packages in the community
Client Interaction: Provide bespoke services and build strong relationships with clients. Act as an advocate for Care4ocus, promoting the business in the community
Communication: Liaise effectively with CCGs, Hospitals, Local Authorities, and other stakeholders. Communicate with a variety of personalities at all levels of the business and build successful relationships.
Leadership and Supervision: Lead, support, and facilitate the nursing and medical needs of service users. Provide leadership to care and support staff, ensuring high-quality care delivery. Deputize for senior management when required. Act as a shift leader to monitor and direct the effective running of the unit or other defined areas of responsibility.
Recruitment and Training: Interview and recommend staff for bespoke care packages. Maintain skills at a current level and undertake further training and development where required. Ensure all staff receive formal supervision and appraisals, identifying opportunities to upskill staff and promote career progression
Documentation and Compliance: Build holistic, person-centred care plans and risk assessments. Ensure compliance with GDPR when handling confidential information and documents. Use Care4ocus’s IT systems for documentation and transcribing MAR charts. Ensure all recording systems and organisational documentation are of a high standard and kept up to date
Quality Assurance: Develop, deliver, and sustain high-quality, client-led, and cost-effective services. Ensure the services meet the requirements of stakeholders and CCG. Identify and share information, concerns, and ideas for improvement with senior leadership.
Team Collaboration: Work with multidisciplinary teams to ensure care is delivered to the highest standards. Ensure service users are at the heart of care delivery and their preferences are supported
Other Duties: Carry out further tasks as delegated by the senior leadership team

To be successful, we think you need:

You must be registered as an RSCN / RN or Dual registered
*
Must have ITU/HDU/PICU/NICU/A&E experience
*
You must have Tracheostomy and Ventilator experience
*
Minimum 5 years post registration experience, 2 years of these in Complex Care environment.

Car Driver with Clear Driving Licence, prepared to travel long distances with possible overnight stays (funded)
*
Having the ability to work calmly under pressure
*
Being able to manage your own diary
