Responsibilities:

· Pay members a friendly health and post check-up follow-up call on a regular schedule.

· Monitor and record unrecorded medication, unrecorded activity for patient record update.

· Promote healthy lifestyles to help lower personal health risks, preventing illness.

· Facilitate effective health care decision-making.

· Improve the quality of care.

· Awareness of the member’s available health and wellness programs, and makes referrals to these programs as appropriate.

· Improve member satisfaction with their health care plan.

· Facilitate appropriate use of health care resources.

· Provides up-to-date health care information to help facilitate members’ understanding of their health problems and possible options.

· Provides assistance by clarifying available options for the member's particular health problem or issue.

· Helps members actively and knowledgeably participate with their provider in their own health care decision-making

Qualifications:

· Active PHRN licensure.

· 3+ years of Telehealth coach experience.

· 3+ years of clinical experience required, specifically in: Disease Management, Emergency Room, Critical Care, Medical/Surgical Care, Oncology.

· 1 year proficiency in Microsoft Office Suite (including Outlook and Microsoft Word).

· Must be willing to work night and start work between 11:00 PM and can choose to work with an 8 hour

· Must have a call-work quiet space at home with Optic Fiber main and secondary internet connection.

· Managed Care experience preferred.

· Previous coaching experience preferred.

· Model candidates have telephone triage nursing experience.
