The Iowa Primary Care Association is actively recruiting a MAT (Medication Aided Treatment) RN Case Manager for a community health center, also called a federally qualified health center (FQHC), in Sioux City, Iowa.

The largest source of comprehensive primary care for medically under-resourced communities and populations.
They have an integrated care model -medical, dental, behavioral health, and pharmacy services, patients can access comprehensive, coordinated services often under one roof.

Position Details:
Schedule: 8AM – 5 PM
In collaboration with provider, performs initial assessment and intake obtaining social, medical, mental health, substance abuse, and medication history.
Provides education and assessment to patients looking to access treatment programs.
Offers referral services for patients looking to access treatment programs.
Assesses and monitors patients in the induction, stabilization, and maintenance phases of treatment.
Provides ongoing management, education, and support of patients in all phases of treatment including; nursing visits, urine toxicology screens, routine labs, medication teaching, monitoring, pill counts, observed dosing, and medication refills.
Follow state and federal guidelines in providing care to opioid-dependent patients in collaboration with license, prescribing providers, and pharmacy.
Complete appropriate documentation for all patient visits and phone calls in a timely manner, ensuring that all notes are complete and easily understood.
May be required to float to the South Sioux City location once MAT services are offered at that location.

Requirements
Minimum of RN license, BSN preferred.
Current BLS and Mandatory Reporter certifications.
Clinical office and case management experience preferred.
Experienced RN with a background in case management who is self-motivated and capable of working with others to improve patient care.
Benefits
Starting salary: $52,000 - $76,000 annually with differential if bilingual.
