Bladen East Health and Rehab, located in Elizabethtown, NC, is seeking a skilled and compassionate Physical Therapist to join our team. As a Physical Therapist, you will work directly with our residents to assess and develop individualized treatment plans to promote mobility, improve strength and function, and enhance overall quality of life. We are dedicated to providing top-notch care and rehabilitation services to our residents, and we are looking for a qualified individual who shares our passion for excellence in patient care.
Responsibilities
Evaluate residents' physical health and functional abilities through comprehensive assessments
Develop and implement individualized treatment plans based on assessment findings
Administer therapeutic exercises, manual therapy techniques, and assistive devices as needed
Monitor and document residents' progress and adjust treatment plans accordingly
Collaborate with interdisciplinary team members to coordinate and optimize resident care
Provide education and guidance to residents and their families on therapeutic exercises and strategies for maximizing function and independence
Maintain accurate and complete documentation of all evaluations, treatments, and progress notes

Requirements
Valid and unrestricted Physical Therapist license in the state of North Carolina
Graduate of an accredited Physical Therapy program
Strong knowledge of physical therapy principles, techniques, and evidence-based practice
Excellent observational, analytical, and communication skills
Ability to work effectively as part of a team
Experience in a skilled nursing facility or long-term care setting is preferred
