Community-Based Palliative Nurse Practitioner

The Palliative Medicine Nurse Practitioner provides consultation in palliative medicine to meet the needs of a patient as requested by their referring physician. The nurse practitioner is a specialist in assessing the patient's condition and developing a plan of care to improve patient understanding of their complex illness, adherence to course of treatment, and quality of life in context of patient’s values. The nurse practitioner also develops a plan of care to manage complex disease-related symptoms including emotion, physical, and existential components. Importantly, the nurse practitioner communicates with all members of the patient’s interdisciplinary care team to insure proper coordination of the best care possible.

Primary Job Responsibilities:
Community-based position: providing palliative medicine to patients wherever they call home
Perform palliative medicine consults utilizing strong assessment and evaluation of the patient in context of their social support system
Collaborate with the patient's physician, other health care team members, the patient and their family in developing realistic care expectations based on goals of care
Facilitate patient and family decision-making regarding treatment decisions
Recommend pharmacological and non-pharmacological interventions
Monitor patient's response to treatment modalities and revise plan as needed
Communicate effectively with patients and families for understanding of diagnosis, prognosis and treatment plans
Develop and maintain effective working relationship with providers, staff, community leaders and organizations to promote positive image; heighten awareness and promote collaboration
Conduct consults and follow up and complete clear and accurate documentation as required in a timely manner
Requirements
Graduate of an accredited school of nursing and nurse practitioner program
Current license in good standing to practice in the Commonwealth of Pennsylvania
Minimum of two years RN or NP experience
ACHPN certification required or be able to obtain certification within 6 months of hire
Maintains clinical and professional competence
Possess a controlled substance license in the Commonwealth of Pennsylvania
Possess the ability to communicate effectively with all members of the care team
Possess good organizational skills
Current driver's license
Reliable transportation and automobile insurance required
Proof of COVID-19 Vaccination or an approved Medical or Religious exemption.
Proof of Influenza (Flu) Vaccination or an approved Medical or Religious exemption.
