Psychiatric Nurse – Part Time
Mohawk's Assertive Community Treatment - ACT Team:
This unique team is an evidence based treatment program that utilizes a mobile, multidisciplinary team of staff to deliver flexible, comprehensive treatment and rehabilitation services to individuals residing in the community with a severe mental illness.
The programs goal is to help individuals achieve stability within their community.
Services are intensive and non-traditional in nature and provided by a team in the community including in the client’s home.
Outreach is a key component of the service model
Responsibilities of the ACT Team Psychiatric Nurse – Part Time: The Assertive Community Team
(ACT) Psychiatric Nurse is a clinical staff member of the ACT community based multidisciplinary treatment team. Nurse is responsible for supplementing all the activities of the Full time Nurse.
Provide assistance to the full time Nurse in the following areas as needed: overseeing all aspects of medication management in compliance with agency procedures and protocols. This includes but is not limited to, administration of injectable medications, managing Clozaril protocols, ordering of medication, overseeing processes for medication distribution, and utilizing a medication step system towards client self-management.
Assessing physical health needs, making appropriate referrals to community physicians, and collaborating with medical providers and pharmacies.
Provide a full range of services that are culturally relevant and recovery based. These services may occur in the home, community or office. Service provision is designed to promote recovery by improving psychiatric symptoms, preventing relapse, teaching skills, and providing direct assistance and securing necessary community resources.
Facilitate client recovery, personal growth; manage symptoms, gain independence and master use of community resources to the best of each client’s ability.
Use strength based methodologies, continually instill hope, develop and maintain trusting and supportive relationships with clients, families, other providers and the community.
Readily respond to all crisis situations and urgent needs using sound clinical judgement in order to ensure the safety and well-being of the individual.
This is a community based position which includes meeting with clients in their homes, community or in the office in addition to providing transportation to appointments.
Conduct person centered comprehensive health assessments, including but not limited to, personal and family medical and psychiatric histories, current medical needs, substance use history, and daily living skills with the purpose of linkage to and collaboration with providers in order to improve health outcomes. Conduct BMI and AIMS assessments as recommended.
Develop and maintain trusting and supportive relationships with clients, families, and community providers. All service provisions shall be judgement free, non-confrontational, yet honest, open and delivered in a professional, caring, supportive and growth producing manner.

Requirements
Skills/Qualifications for the ACT Team Psychiatric Nurse – Part Time:
Licensed Registered Professional Nurse by New York State.
Bachelor’s Degree in nursing preferred.
Three years’ experience in a psychiatric setting, with at least one year experience in an outpatient setting is preferred.
Must have a strong commitment to the rights and abilities of each person with a severe mental illness to live in the community, work, and have access to continuous support and services to enhance wellness and recovery.
Must be able to effectively work with all people and demonstrate skills and competence to establish supportive and trusting relationships with all individuals.
Mohawk’s programs and offices are multi-level with stairs only. All employees must be able to travel to all levels of the facility in a safe and reasonable time frame.
A valid and insurable driver’s license
Benefits
Salary and Schedule for the ACT Team Psychiatric Nurse – Part Time:
$40.16/hour
Schedule:16 hours per week (no benefits). Work hours are within the client service hours of Monday-Thursday, daytime hours.
