Summary:
LVN works closely with each of the Clients and Medical staff, on an individual basis providing assistance in the areas of, medication observation and medical appointment scheduling, as well as overall client health, in an intensive inpatient health care facility.
Job Duties:
Observation, documentation, audit functions, and record keeping of all observed/inventoried/destroyed/prescribed medications.
Assessment of patients with undiagnosed or diagnosed conditions, and assisting in ascertaining clinical medication management with Nurse Practitioner and Medical Director.
Observation, documentation, and record keeping of client health status.
Works closely with Nurse Practitioner/Medical Director/ LMFT/ and Case Managers on client health status and presenting conditions.
Manages complete Medication Observation Record Sheets and system.
Set all medical appointments for clients including medical, dental, and psychiatric as needed.
Education and counseling for clients and other organizational health care providers on optimal use of medicines (e.g., proper use, avoidance of over medication)
Requirements:
Active license and able to show proof
Preferred 2 years of experience in substance abuse Detox, Pharmacy, Mental Health, or behavioral health services setting.
Clean DMV record and/or in accordance with company insurance requirements.
Valid Driver’s License.
Must be open to evenings and weekends
