Description:
An exciting opportunity has arisen, supporting an organisation as a Head of Community and Day Services.

The post will be mostly remote working with some onsite needed, on an interim basis for 6 – 12 months, on a competitive salary.
The post holder will have clinical operational responsibility for Clinical Community Services and the Day Service delivered by the organisation.

Main Responsibilities:

Directly line manage and supervise all clinical staff within the function;
Take leadership of strategic development of the Community Service by working closely with the Community Service Lead Nurse and Day Service, and collaboratively work and develop with other clinical services;
Work alongside the Community Service Lead Nurse and Day Service to ensure high quality evidence-based service is provided;
Provide strategic leadership for the development of the Community Services, including the Day Service, to meet the needs of patients, and their significant others, who require specialist palliative care and/or end of life care and ensure the best utilisation of resources;
Demonstrate clinical excellence, displaying sound understanding of evidence-based symptom management and holistic support;
Be a professional role model, develop, lead and co-ordinate the provision of high quality, effective care that is safe, minimises risks and promotes personalisation of care;
Always ensure that the needs of the patient are placed at the centre of all clinical decision processes and care delivery;
Ensure patients are actively involved in decision making whenever possible;
Be professionally and managerially accountable in association with the Community Service Lead Nurse for the quality of nursing care provided by the Community Service and Day Service;
Maintain awareness of research and relevant findings and in conjunction with the multi-professional team, work towards these being applied to practice;
Participate in education programmes as required;
Provide feedback from attended education programmes or study days to the MDT;
Utilise awareness of the cost-effective application of the financial and physical resources within the organisation;
Responsible for the identification of equipment necessary for service provision;
Ensure effective systems are followed to integrate risk management and Clinical Governance to reduce clinical risk and improve patient safety;
Make certain that all serious and untoward incidents and major risks are notified through the appropriate reporting mechanisms;
Devise, in conjunction with the line manager, a personal development plan in line with the organisation objectives and personal needs.
A successful candidate will have:

Current NMC registration – First Level Adult Registered Nurse;
Educated to degree level or equivalent experience;
Evidence of ongoing professional development;
Experience of working within community services;
Appropriate demonstrable post registration experience;
Experience of leading staff and influencing care standards;
Clinical Understanding of Palliative Care Services;
Enthusiastic, dynamic, tactful.
