Seasoned Recruitment is currently seeking compassionate and highly skilled Home Health Case Managers to join our team in Cleveland and surrounding areas. As a Home Health Case Manager, you will play a vital role in coordinating and managing the care of patients receiving home health services.
In this role, you will be responsible for assessing patients' needs, developing individualized care plans, coordinating with healthcare providers and community resources, providing education to patients and their families, and ensuring that care is delivered in accordance with the established standards and guidelines.

Requirements
To be considered for this position, you must meet the following requirements:
Hold a valid and unrestricted license as a Registered Nurse (RN)
Have a minimum of 2 years of experience as a Case Manager in a home health or similar setting
Possess strong assessment and care coordination skills
Excellent communication and interpersonal skills
Ability to work independently and manage multiple cases
Proficient in using electronic medical record systems
Have a reliable means of transportation
Be willing to travel within the designated service area in Cleveland and surrounding areas
