Elite Staffing Network is seeking a Care Manager- RN for Monogram Health in Saint Paul, MN.  
qualifications
requiredValid Registered Nurse (RN) license/certification in Minnesota (MN)
2+ years experience in any of the following:
Dialysis Care
Home Health Care
Hospice
Case Management (CM)
Are you seeking a full-time position? (yes)
Are you comfortable traveling to patient's homes? (yes)
The work schedule is M-F from 8 AM - 5 PM. Are you available and comfortable with this schedule? (yes)
You will be travelling within a one-hour travel radius from the main location within which you will be visiting patients in-home. The job is 3-4 days on the road visiting patients and 1-2 days telehealth. Are you comfortable with this and willing to put in the road time? (yes)
preferredAre you bilingual English/Spanish? (yes)
